On May 11, 2023, the Commonwealth of Massachusetts declared an end to the COVID-19 public health emergency. Mask usage at medical facilities including our offices will be optional. At your request, we can supply you with a face mask during your visit. If you are experiencing symptoms of an upper respiratory infection (cough, fever, sore throat) we kindly request that you contact our office in advance and we will be happy to reschedule your appointment. - Driver’s License or a valid ID
- Insurance information
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
- List of medications (if any)

Telephone the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list.
