Please arrive fifteen minutes early for your initial appointment at NWOA. We will ask you to fill out paper work and verify contact and insurance information.
Please be sure to bring your insurance card(s) with you to your appointment. If your insurance requires it, please also obtain and bring a referral from your primary care physician to your visit.
Your co-pay is required at the time of your appointment.
In an effort to help you save time, we offer you the ability to download, print and complete the necessary forms prior to your appointment with us. Remember to bring the completed forms to your first visit.
Prescriptions and Medications
Many orthopedic conditions require the use of medications to control pain and inflammation. If you have any questions regarding your medication, or if you need a medication refilled, please feel free to call us during our regular office hours.
NO PRESCRIPTIONS WILL BE FILLED AT NIGHT OR ON WEEKENDS.
To ensure that you have your medication when you need it, YOU MUST ALLOW 24 HOURS FOR MEDICATION REFILLS TO BE PHONED TO YOUR PHARMACY (narcotic medications cannot be phoned in and require an office visit.) Please have the telephone number of your pharmacy available when you call for a refill.
If you should sustain an injury at your workplace, we would be happy to evaluate and treat your injury. You must provide our billing department with the necessary forms from your employer and your workers’ compensation insurance carrier. If we do not receive the information, you will be responsible for payment at your visit.
Motor Vehicle Accidents/Personal Injuries
We are happy to care for those injured in an accident. In the case of a motor vehicle accident, patients must provide our billing department with all motor vehicle insurance information as well as health insurance information in advance of their appointment. This will permit our billing department to secure confirmation that the motor vehicle insurance company or your health insurance will cover patient’s medical expenses, which were incurred as a result of the accident.
For personal injury or slip and fall accidents, payment is due from the patient on the day of the visit and is not contingent upon the status or outcome of the liability case.
We understand that medical insurance can sometimes be intimidating and confusing to the patient. Our staff will do their best to help you understand the insurance process and will be glad to answer any questions that you might have. Some companies pay fixed allowances for certain procedures, and others pay a percentage of the charge.
It is your responsibility to pay any deductible amount, co-insurance, or any other balance not paid by your insurance company. If your insurance carrier requires that you obtain a referral from your primary care physician, it is your responsibility to provide us with a valid referral number at the time of your visit. If no referral number is available, you will be asked to sign a waiver stating that you are aware of your financial responsibility for our service if we do not receive your insurance referral from your primary care physician.
We are available for emergency care 24 hours/day.
If your call is during our regular office hours, our support staff will notify one of the doctors or physician’s assistants of your situation and you will be advised accordingly.
If you should have an emergency outside of our regular office hours or on a weekend, a physician can be reached through our answering service by dialing one of the phone numbers listed above.
